The inspector’s account

Licensing Program Analyst (LPA) Iby conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified herself to and discussed the purpose of the visit with Program Administrator Leilanie Baltazar-Charlot. According to the facility’s license, there may be a maximum of one hundred eighty (180) developmentally disabled adults at any given time, forty (40) of whom may be non-ambulatory. Program has a RHCP for wound and diabetes. One client with a wheelchair has been approved for a lap belt, chest straps, and foot straps.

LPA and Program Administrator toured the interior and exterior of the facility. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Doors, windows and screens, sinks, and toilets were in working order. Hand hygiene supplies and Personal Protective Equipment were present. The facility had sufficient space and equipment to facilitate meetings and client activities. Hot water was measured at at 115 degrees F. Medication was observed locked and labeled.

No pools or bodies of water on the premises. There were no sharp objects, toxic chemicals/poisons, fireplaces, or open-faced heaters accessible to clients. Per the Program Administrator, no firearms or ammunition are kept at the facility. Smoke alarms, carbon monoxide detectors, fire extinguishers, and facility telephone are all present and working. First aid kits were readily accessible.

Staff and client files reviewed contained required documentation. Required licensing postings were observed in visible areas of the facility. LPA will provide facility with an updated license to reflect exceptions and waivers. No deficiencies were observed or cited during today's visit.

An exit interview was conducted with Program Administrator, to whom a copy of this report, and the Applicant/Licensee Appeal Rights (LIC9058 03/22) were provided to.
