Facility Evaluation Report
Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced Case Management - Incident visit. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Administrator Shanta Haines. LPA also met with Licensee Rukhsana "Roxy" Gasque, who arrived later during the visit.
Today's visit was in response to an LIC624 Incident Report, which licensee self-submitted to the CCLD San Diego Regional Office (received on 04/10/2023). According to the LIC624: On 04/03/2023, Staff #1 (S1) and Staff #2 (S2) took Client #1 (C1), Client #2 (C2), Client #3 (C3), and Client #4 (C4) to a casino. [See LIC 811 Confidential Names List for a description of person identifiers used in this report]. While at the casino, S1 and S2 did not provide needed supervision to clients.
During today’s visit, LPA briefly toured the day program facility and performed a welfare check on the client participants, finding no immediate safety concerns. LPA also reviewed pertinent care and administrative records and interviewed relevant staff.
According to C1’s LIC602 Physician’s Report, they were diagnosed with “Developmental Delay” and “Epilepsy,” and their doctor determined that C1 required staff supervision when in the outside community for their safety. According to C2’s LIC602 Physician’s Report (dated 02/09/2023), they were diagnosed with “Mild Intellectual Disability” and “Epilepsy,” and their doctor determined that C2 required staff supervision when in the outside community for their safety. According to C3’s LIC602 Physician’s Report (dated 02/24/2023), they were diagnosed with “Mild Intellectual Disability” and “Autism,” and their doctor determined that C3 required staff supervision when in the outside community for their safety. According to C4’s LIC602 Physician’s Report (dated 08/23/2022), they were diagnosed with “Schizophrenia,” “Bi-polar Disorder,” and “Mental Retardation,” but their doctor determined that C4 could safely self-manage themselves in the outside community.
