Facility Evaluation Report

Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced Case Management visit for a self-reported incident. LPA Lopez identified herself and was granted entry by the Day Program Manager, Virginia Maulupe. LPA stated the purpose of the visit and reviewed the basic elements of today’s visit with the Administrator, Shanta Haines, who later arrived.

This visit was in response to a self-reported incident that was received by the San Diego Regional Office on 09/02/2025. The facility reported an incident that transpired between client #1 (C1 – see confidential names list) and client #2 (C2 – see confidential names list) around lunchtime on 08/25/2025.

During today’s visit, LPA requested and obtained relevant Day Program records pertinent to this incident. LPA informed Shanta Haines, Administrator, that, at this time, this case may require additional telephone calls or visits relating to this incident. No deficiencies were observed or cited during this visit.

An exit interview was conducted with Shanta Haines, Administrator, to whom a copy of this report, the LIC811 Confidential Names List, and the Licensee/Appeal Rights (LIC9058 01/16) were provided at the conclusion of the visit. The signature below confirms the documents were received.
