Facility Evaluation Report
Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA identified herself upon arrival and was greeted by Executive Director Jessica Zepeda.
The purpose of the visit was discussed with Executive Director Zepeda and Business Office Director Ellen Arguello.
According to the facility license, the approved capacity is 105 residents, all non-ambulatory, with up to seven bedridden residents permitted on the first floor only. At the time of inspection, facility records indicated 89 residents in care, including 45 non-ambulatory residents and 23 residents in the memory care unit. No residents were bedridden.
During the visit, the LPA toured the interior and exterior of the facility with Executive Director Zepeda, inspected common areas and a sample of resident bedrooms, reviewed staff and resident records, and conducted private interviews with staff and residents. All reviewed records contained the required documentation.
The facility was clean, sanitary, and in good repair. Pathways were clear of obstructions and slip hazards. Resident bedrooms contained required furnishings. Doors, windows, screens, toilets, and showers were in working order. Adequate linens, hygiene supplies, and personal protective equipment were available.
