Facility Evaluation Report

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced visit to complete a Required Annual Inspection. During the inspection, the LPA also conducted a case management visit.

The LPA was welcomed by Housekeeper Ana Laura Munoz, identified herself, and explained the purpose of the visit.

The LPA also spoke with Administrator Ernesto Sandoval via telephone.

During today’s visit, LPA Garcia-Centeno discussed the following:

1. Client Rooms : Client rooms must be fully prepared prior to admission. All required furnishings were present; however, the rooms showed signs of occasional use. Clothing, pets, and other personal items were observed during the visit.

2. Fire Extinguisher : The fire extinguisher did not have an updated service tag. The existing tag displayed a service date of 01/21/2025. The LPA confirmed that a new fire extinguisher had been purchased but had not yet been placed at the facility at the time of the visit. The Administrator stated that the new extinguisher will be installed as required.

3. Facility Profile : The facility profile information is outdated, and the phone numbers listed are not current. The Administrator will submit a request to update the facility profile.

4. Voicemail Greeting : The facility’s voicemail greeting needs to be updated.

No deficiencies were observed or cited during today’s annual inspection.

An exit interview was conducted with Administrator Ernesto Sandoval. A copy of this report, along with the Licensee/Appeal Rights (LIC 9058 03/22), was provided during today’s visit .
