The inspector’s account

Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced Case Management Visit to cite a deficiency identified during a separate complaint investigation. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Manager Rzecore "Crown" Sorenson .

LPA observation and staff interviews showed: Staff #2 (S2) was employed in direct-care role during December 2024. LPA met S2 on duty during LPA’s 12/03/2024 complaint investigation visit. S2 at that time told LPA they were working “full-time.” Interviews of staff showed S2 later voluntarily resigned from facility employment on/around 12/24/2024.

A subsequent review of CCLD’s Guardian Database showed that Licensee did not ensure that S2 had an approved criminal-record background clearance or exemption, prior to employing S1 at the facility. The preponderance of evidence showed S2 had worked more than five (5) days at the facility without such a clearance/exemption. LPA conferred with Licensee to see if they might have a CCLD background clearance letter for S2, but Licensee did not.

One (1) deficiency was cited per California Code of Regulations, Title 22 (refer to the attached LIC 809-D page). Since the deficiency pertains to criminal record clearance, by statute, an immediate civil penalty of $500 was also assessed/charged to Licensee (refer to the attached LIC421-BG page). A Plan of Correction was jointly developed with the Licensee.

An exit interview was conducted with Manager Rzecore "Crown" Sorenson , to whom a copy of this report, the LIC 809-D page, the LIC 421-BG page, and the Licensee/Appeal Rights (LIC9058 03/22) were provided.
