Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced visi. LPA met with Licensee Liberty Cruz-Nelson, who arrived shortly after.
During the visit, LPA Garcia-Centeno discussed with Licensee Cruz-Nelson the procedures for requesting age exceptions approval from CCL. Review of records indicated that one client was over the age limit. Facility is licensed for adults between the ages 18-59.
An exit interview was conducted with Liberty Nelson, to whom a copy of this report, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during today's visit.
