Facility Evaluation Report
Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced Case Management Visit to cite a deficiency identified during a separate complaint investigation. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Caregiver Carmen De Jesus. LPA then met with Licensee/Administrator Rhonda Epps and Co-Administrator Patricia Daniels, who arrived shortly after.
Records and interviews showed that on 07/23/2025, Licensee received constructive knowledge that Client #1 (C1) had obtained a pumpkin-cutter tool from the facility’s garage, then used this object to try to slit their left lower forearm while they were at R.E. Care Homes LLC. [See LIC811 Confidential Names List for a description of person identifiers used in this report.] The CCLD was not notified of this incident by the next business day, and no written report was submitted to CCLD within seven (7) days of incident occurrence, as required.
Regarding reporting requirements, one (1) deficiency was cited per California Code of Regulations, Title 22 (refer to the attached LIC 809-D page). A Plan of Correction was jointly developed with the Licensee.
An exit interview was conducted with Licensee/Administrator Rhonda Epps and Co-Administrator Patricia Daniels, to whom a copy of this report, the LIC 809-D, the LIC811 Confidential Names List, and the Licensee/Appeal Rights (LIC9058 03/22) were provided.
