Complaint Investigation Report
Interview with facility staff revealed that there had been no evictions given in the past month. Interview with outside source (OS1) revealed that there was a care conference where a discussion of revaluating if facility is able to meet R1’s needs was had. OS1 stated that an eviction was not given and had no concerns for facility issuing illegal evictions. OS1 stated that R1 has returned to the facility. Interview’s with facility staff corroborated what OS1 reported. Facility staff stated that no eviction was issued to R1.
Based upon the foregoing, the above listed allegations are unsubstantiated. This finding means that the preponderance of the evidence standard has not been met and the allegations are not valid. No deficiencies were cited today.
