Licensing Program Analyst (LPA) conducted an unannounced visit to conduct a case management visit in response to death report submitted by facility dated 01/07/2026. LPA disclosed the purpose of the visit with Executive Director Kim Stratman.
During the visit, LPA reviewed and obtained the required records. No deficiencies were cited in accordance with the California Code of Regulations. An exit interview was conducted with Executive Director Kim Stratman, to whom a copy of this report, and the Licensee/Appeal Rights (LIC 9058 03/22) were provided to.
