Facility Evaluation Report

Licensing Program Analyst (LPA) Alyssa Ramirez conducted an unannounced visit to conduct a case management visit in response to incident report submitted by facility dated 7/31/2024. LPA disclosed the purpose of the visit with Director of Nursing Ivonne Nochez.

During the visit, LPA reviewed records and spoke to staff and clients. Based on today's visit, no deficiencies were observed or cited during this visit.

An exit interview was conducted with Nochez to whom a copy of this report along with the Licensee/Appeal Rights (LIC9058 03/22) were provided at the conclusion of the visit. The signature below confirms the documents were received.
