Facility Evaluation Report
Licensing Program Analyst (LPA) Marisela Garcia-Centeno made an unannounced visit to conduct a Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Caregiver, Lluvica Cabeza. LPA spoke on the telephone with Licensee/Administrator Jeff Jackson. All staff present in the facility had current criminal background clearances on record. LPA introduce herself as their new LPA and had the opportunity to answer any questions.
According to the facility’s license, the facility has a maximum capacity for four (4) clients, of whom all must be ambulatory. Three (3) Clients were out in the community and one (1) clients was sleeping during the visit.
Per Administrator, Jackson, CCL conducted the annual inspection on February 3, 2026. LPA conducted a post licensing inspection and health and safety visit.
During today's visit, LPA Garcia-Centeno conducted a quick tour of the facility and interviewed staff.
No violations were observed during today's visit.
An exit interview was conducted with Caregiver, Cabeza, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided during today’s visit.
