Facility Evaluation Report

On 5/4/2026, Licensing Program Manager (LPM) Robyn Clark met with the licensee over virtual conference to discuss the status of the facility license. In attendance were Bertha Rodriguez, Licensee and Administrator and Oyewole (Joseph) Jo se, representative for the buyer and prospective Licensee.

During the meeting, Mr. Jose clarified the chain of events and communications with the Centralized Application Bureau. The status of the license was clarified, and a review of the Secretary of State Website also indicated that the Corporation was active and in good standing. LPM provided consultation on Title 22 regulations and reminded both parties of the requirement to ensure compliance with all regulations while the change of ownership is pending.

Exit interview conducted. This report along with Licensee Appeal Rights provided via email to both parties at the conclusion of the meeting, with confirmation of receipt verified via email .
