Facility Evaluation Report

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced visit to conduct a Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Property Owner, James Ducharme.

During the visit, LPA confirmed that facility was closed and no clients were present at the facility.

Per Ducharme, License, Eduardo Gonzalez gave a 90 day notice in January 2026. Licensee moved out on April 10, 2026, per Ducharme. Licensee, Eduardo Gonzalez moved to Florida.

A case management visit was performed on January 28, 2026 to close the facility.

The facility was under construction and renovation.

An exit conference was conducted, and a copy of this report was provided to Property Owner, Ducharme.
