The inspector’s account

Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced Case Management - Incident visit. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with DSP Tara Lockhart. LPA then met with Licensee Raquel Preece, who arrived later during the visit.

Today's visit was in response to a Special Incident Report (SIR) and a SOC341 Report of Suspected Dependent Adult Abuse, which licensee self-submitted to the CCLD San Diego Regional Office (both received on 07/20/2023). The reports described the behaviors of Client #1 (C1) towards Client #2 (C2) and Staff #1 (S1) during a 07/18/2023 incident. During today’s visit, LPA performed a brief facility tour and welfare check. All four (4) clients in care appeared calm and agreeable in each others' presence, and none expressed fear or concern about any house mate. LPA also reviewed pertinent records and interviewed relevant staff.

Interviews revealed: during the incident in question, staff quickly intervened to separate C1 from C2 and to protect clients’ personal rights. C1 was aware of how they acted during the incident and emphasized to LPA their commitment to respecting others. Staff reported that since the incident and as of the date of LPA’s visit, there have been no subsequent challenging behaviors from C1.

No deficiencies were cited for the above incident. Also, no deficiencies were observed or cited during today's visit.

An exit interview was conducted with Preece, to whom a copy of this report, the LIC811 Confidential Names List, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
