Facility Evaluation Report

Licensing Program Analyst, (LPA), David Roman conducted a Case Management visit. LPA was greeted and allowed entry into the facility by Licensee, Raquel Preece and Derek Hampton. LPA discussed the purpose of the visit, to verify there are no clients in care, due facility closure.

In 05/04/2026, Licensee provided verbal notice of facility closure. In addition, Community Care Licensing received written notice that the facility will be closing on 06/26/2026, due to the licensee no longer interested in operating the facility. Licensee met with San Diego Regional Center on 06/25/2026 along with the four (4) clients in care, regarding closure and new placement. All clients responsible parties were made aware and agreed to relocation 60 days prior. As of 06/25/2026, all four (4) clients were relocated. LPA toured the facility and verified there are no clients in care. Licensee provided written proof of closure, along with original license. Facility is ready for closure. No deficiencies were issued.

An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 01/16) was provided to Licensee, Raquel Preece and Derek Hampton whose signature below confirms receipt of this report and their rights.
