Facility Evaluation Report

Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced Case Management Visit to cite a deficiency identified during a separate complaint investigation. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Caregiver Pio Abille. LPA then met with Manager Pia Abille, who arrived shortly after.

During LPA’s 09/16/2025 site visit, he observed multiple houseflies in the facility’s kitchen and dining room areas. [These flies were not present during LPA’s prior recent visits on 08/22/2025 and 07/17/2025.] One of the six clients in care also made an unsolicited comment to a housemate about the houseflies, which LPA incidentally overheard.

One (1) deficiency was cited per California Code of Regulations, Title 22 (refer to the attached LIC 809-D page). A Plan of Correction was jointly developed with the Licensee.

An exit interview was conducted with Manager Pia Abille, to whom a copy of this report, the LIC 809-D, and the Licensee/Appeal Rights (LIC9058 03/22) were provided.
