COMPLAINT CONTROL NUMBER: 08-AS-20230517143053
Licensing Program Analyst (LPA) Natasha Persaud conducted an unannounced visit to commence a complaint investigation. LPA identified herself and discussed the allegation mentioned above with Staff, Pia Abille.
During today's visit, LPA briefly toured the facility, interviewed staff and clients. It was alleged the facility is in disrepair. It was reported the floor surface are dirty and sticky, and the shared bathroom has mold on the ceiling. LPA observed debris on the floor throughout the facility, dirty stained walls, and mold spots on the ceiling above the shower. The smoke detector in the client room located near the water heater is chirping, as the battery needs to be replaced. Staff stated they clean daily. Today the facility was observed in disrepair. Based on LPA’s observations and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California code of Regulations, Title 22, Division 6 & Chapter 1 is being cited on the attached LIC 9099D. An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 01/16) were provided to Staff, Pia Abille whose signature below confirms receipt of these rights.
