Facility Evaluation Report
Licensing Program Analyst (LPA) Ramon Serrano, conducted an unannounced Case Management Visit. The facility file was reviewed prior to the visit. LPA met with Director of Resident Services Corinna Norton and discussed the purpose of the visit. All staff present have a current criminal record clearance.
LPA conducted a health and safety visit, interacted with staff and resident's in care and obtained facility records.
No deficiencies were cited or observed on this date.
An exit interview was conducted with Corinna Norton. A copy of this report along with licensee rights (LIC 9098, 01/16) was provided to Corinna Norton whose signature below verifies receipt of these rights.
