Facility Evaluation Report
Licensing Program Analyst (LPA) Jose De La Cruz conducted an unannounced visit to conduct an annual inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Licensee Eddie Nguyen. The facility's license shows a maximum capacity of four ambulatory clients, ages 18 through 59.
The facility currently does not have clients in care.
LPA arrived at 1:00 pm, and toured the facility with Licensee, the interior and exterior and inspected four residents’ rooms, garage and one bathroom. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities.
Per Licensee, no firearms or ammunition are kept at the facility. Carbon monoxide detectors, and facility telephone were all in working order. There is a pool and a fireplace on the premises. First aid kit was complete and readily accessible.
No deficiencies were cited by the California Code of Regulations. An exit interview was conducted with Licensee Eddie Nguyen, to whom a copy of this report, and the Licensee/Appeal Rights (LIC9058 03/22) were provided during today’s visit.
