Facility Evaluation Report

Licensing Program Analyst (LPA), Ramon Serrano, conducted an unannounced collateral visit regarding a complaint investigation for another facility. LPA met with Caregiver Natalie Palma and discussed the purpose of the visit. Administrator Mary Ann Gutierrez arrived shortly after.

During the visit, LPA conducted staff and resident interviews and obtained facility records.

An exit interview was conducted with Mary Ann Gutierrez. A copy of this report along with Licensee Rights was provided to Mary Ann Gutierrez whose signature below verifies receipt of these rights.
