The inspector’s account

Licensing Program Analyst (LPA) Janet Ngallo conducted an unannounced Case Management visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Case Managers Patricia Ochoa and Adela Martinez.

Today's visit was in response to Licensee’s self-reported SOC 341 between Client 1(C1) and Client 2(C2), received at the CCLD San Diego Regional Office on 01/08/2026.

During today’s visit, LPA briefly toured the Adult Day program site and collected pertinent records. LPA performed a welfare check on the clients in care, finding no immediate safety concerns.

An exit interview was conducted with Case Managers Patricia Ochoa and Adela Martinez to whom a copy of this report along with the Licensee/Appeal Rights (LIC9058 03/22) were provided at the conclusion of the visit. The signature below confirms receipt of these documents.
