The inspector’s account

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. Upon arrival, LPA was greeted by Caregiver Joana Reasonada, to whom LPA identified herself and explained the purpose of the inspection. Licensees Bernard (“Bernie”) Aquino and Felicidad (“Pheli”) Aquino arrived during the visit. The Administrator certificate is valid through July 2027. All staff present had criminal record clearances.

According to the facility license, the maximum capacity is six (6) clients, who may be ambulatory or non-ambulatory, but not bedridden. During today’s inspection, there were five (5) clients in care; one client was non-ambulatory. Two additional clients were out in the community.

LPA interviewed all staff and clients present. LPA reviewed care records for all clients and personnel files for all active staff. LPA, accompanied by staff, toured the interior and exterior of the facility, including all common areas and client bedrooms.

The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Client bedrooms contained required furnishings. Doors, windows and screens, toilets, and showers were functional. Adequate linens, hygiene supplies, and Personal Protective Equipment (PPE) were available. The facility had sufficient space and equipment for dining, laundry, visitation, meetings, and client activities.

The ambient indoor temperature was comfortable at 74°F.
