Facility Evaluation Report
Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced Collateral Visit. LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Caregiver Junior Catalino. LPA then met with Administrator Dulce Antonio and Manager Jerijan Antonio, who arrived later during the visit.
During today’s visit, LPA conducted staff and client interviews to aid in an investigation involving a different licensed care facility.
No deficiencies were observed or cited during today's visit.
An exit interview was conducted with Administrator Dulce Antonio and Manager Jerijan Antonio, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided.
