Facility Evaluation Report
Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced Case Management Visit . LPA was welcomed by, identified himself to, and discussed the purpose of the visit with Lead Caregiver Elizabeth De Guia. LPA also met with Licensee/Administrator Henrietta “Candy” Chong, who arrived later during the visit.
During the visit, LPA Garcia-Centeno discussed the procedures and requirements to submit the over age limit exception. Facility is licensed as an ARF and currently there are two (2) clients over the age of 59. One more client turns 60 in December 2026.
No violations were cited during the visit.
