The inspector’s account

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced visit to conduct a Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by, identified herself to, and discussed the purpose of the visit with Lead Caregiver Elizabeth De Guia. LPA later met with Licensee and Administrator Henrietta “Candy” Chong, who arrived during the visit.

According to the facility’s license, the facility has a maximum capacity of four (4) clients. All clients may be ambulatory or non-ambulatory, and one (1) client may be bedridden in Bedroom #1 only, per the approved facility sketch. Based on LIC 602 Physician’s Reports, staff interviews, and LPA observation, there were four (4) clients in care at the time of the inspection. Two clients were non-ambulatory. The facility’s license does not include endorsements for delayed-egress doors or a secured perimeter, and neither were observed during the inspection.

During the visit, LPA interviewed multiple staff members and reviewed care records for all clients, as well as personnel and training files for all current staff. LPA also toured the interior and exterior of the facility and inspected all common areas and bedrooms. The facility’s ambient temperature was observed to be 71°F. At the time of LPA’s inspection, hot water at taps accessible to clients was within regulatory limits. The kitchen sink water temperature measured 113.6°F.

The facility was clean, well-maintained, and free of obstructions or slip hazards. Client bedrooms contained the required furnishings, and doors, toilets, and showers were observed to be operational. Adequate linens and hygiene supplies were available.
