Facility Evaluation Report
Licensing Program Analyst (LPA) Alyssa Ramirez conducted an unannounced visit to commence a Required Annual Inspection. The facility file was reviewed prior to the visit. LPAs were welcomed by, identified themselves to, and discussed the purpose of the visit with Licensee Alejandra Basurto.
During today’s visit, LPA toured the facility and reviewed staff and client records. No deficiencies were cited during today’s visit. Due to technical issues, a return visit on a subsequent day is needed to complete the annual inspection.
An exit interview was conducted with the Basurto, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
