Facility Evaluation Report

Licensing Program Analyst (LPA) Sabel Martinez conducted an unannounced Case Management visit. The LPA introduced himself and disclosed the purpose of the visit to Director of Operations Sherry Hockendorf. Director of Operations Cecelia Steinbach arrived during the visit and assisted the LPA.

The visit was in response to an Incident Report submitted to the Department (received on 4/7/25) regarding Client # 1 (C1). During the visit the LPA toured the facility, conducted interviews and secured records. There were no immediate health and safety concerns, but additional visits and follow ups may be necessary. No deficiencies were cited on today's date.

An exit interview was conducted with Hockendorf and Steinbach, to whom a copy of this report, and Licensee Rights (LIC 9058), were provided.
