Facility Evaluation Report

Licensing Program Analyst (LPA) Hannah Rodgers conducted an unannounced visit to conduct a case management visit in response to two (2) incident reports submitted by facility dated 12/02/24 & 12/03/24 regarding Client #1 (C1). LPA disclosed the purpose of the visit with Administrator Melissa Martinez.

During the visit, LPA reviewed records, conducted a welfare check, and spoke to staff and clients. Based on today's visit, no deficiencies were observed or cited during this visit.

An exit interview was conducted with Administrator Martinez to whom a copy of this report along with the Licensee/Appeal Rights (LIC9058 03/22) were provided at the conclusion of the visit. The signature below confirms the documents were received.
