Licensing program Analyst (LPA) Kennedy conducted an unannounced visit to the facility. LPA identified herself upon entry. LPA met with Pamela Talmantes, Resident Services Director. . The purpose of today's visit was to sign an amended a prior report that was found to contain some erroneous information.
The only business conducted today was the was the signing of the amended report.
An exit interview was conducted with Pamela Talmantes, Resident Services Director. , of a copy of the amended report, this report and the Licensee Appeal Rights (LIC9058 3/22) were left at the facility.
