Facility Evaluation Report
On 09/12/25 Licensing Program Analyst (LPA) Javina George made an unannounced case management Incident visit in regards to an Unusual Incident/Injury report that was received on 09/09/25. LPA met with Administrator Eloisa Karatas and informed her of the purpose of visit.
The department received an incident report which revealed that on 09/08/25, Resident #1 (R1) returned from a home visit on 09/07/25. The next day R1 had discharged the fire arm, as there was a sound heard, smell burn and half of a bullet observed on the floor by facility administrator. Through the administrator's investigation revealed that R1 brought the hand gun back from their home visit on 09/07/25. There were no injuries sustained by R1, other residents or facility staff. The gun was removed from the premises on 09/09/25, by R1s responsible party, and the necessary cross reports were completed.
During today's visit LPA verified with R1s responsible party that they did in fact pick up the hand gun. In addition per the administrator there are no other known guns or ammunition on the premises.
There were no health and safety concerns observed during today's visit.
An exit interview was conducted where a copy of this report was reviewed and provided to Eloisa Karatas, administrator.
