Facility Evaluation Report
Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced Required Annual Inspection. Prior to the visit, the facility file was reviewed. Upon arrival, LPA was greeted by Administrator Martha Caro Bravo, to whom she identified herself and explained the purpose of the visit. All staff present had current criminal record clearances. The administrator certificate is currently in pending status.
According to the facility’s license, the maximum capacity is four (4) clients, all of whom may be non-ambulatory. During today’s inspection, three (3) clients were in care. Per physician reports, all three are non-ambulatory. At the time of the visit, all clients were out in the community.
LPA, accompanied by Administrator Caro, conducted a tour of the interior and exterior of the facility. The facility was observed to be clean, sanitary, and in good repair. Pathways were free of obstructions and slip hazards. Clients' bedrooms contained the required furnishings. All doors, windows, screens, toilets, and showers were in good working order. Extra linens, hygiene supplies, and Personal Protective Equipment (PPE) were available. The facility provided adequate space and equipment for dining, laundry, visitation, meetings, and client activities.
The facility’s ambient temperature was compliant at 78°F. Hot water temperature at the kitchen sink was 109°F, within the regulatory range.
Food supply met requirements: at least two days of perishable and seven days of non-perishable food were present and properly stored. Cooking and dining equipment and utensils were available. No sharp objects, toxic chemicals, or open-faced heaters were accessible to clients. Medications were properly labeled and stored in locked areas. (continue at LIC809C)
