Facility Evaluation Report
Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted a case management visit during an annual required inspection. LPA met with Administrator, Brian Seacord and discussed the purpose of the visit .
During the visit, LPA Garcia-Centeno reviewed infection control procedures in detail due to the COVID outbreak that began Monday, September 29, 2025. Administrator has been working with the Department of Public Health since the onset of the outbreak. During today's visit one client is in isolation and two staff members were out with COVID.
No violations were cited during today's visit.
An exit interview was conducted with Administrator, Brian Seacord to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
