Facility Evaluation Report

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced Required Annual Inspection. During an inspection, LPA conducted a case management visit as well.

During today's visit, LPA reviewed the following regulations with Administrator Bernarte:

Reporting requirements regarding construction at the facility that may have an impact on the clients under care. Buildings and grounds regulations.

Violations were cited under the annual report, LIC809D during the annual inspection.

An exit interview was conducted with Administrator Bernarte. A copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided during the visit.
