Facility Evaluation Report

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted a case management visit after

completing the annual inspection. LPA identified herself and was granted entry by Caregiver, Theresa Sy. Administrator, Wa Lai Bernarte, arrived later during the visit and LPA disclosed the purpose of the visit.

During the case management visit LPA Garcia-Centeno discussed the age requirement for clients living at the ARF. The licensee agreed to submit formal request to CCL for age exception. LPA outlined the procedures for submitting a request to CCL.

No violations were cited during today's visit.

An exit interview was conducted with Licensee, WaLai Bernarte, to whom a copy of this report, and the

Licensee/Appeal Rights (LIC9058 03/22) were provided at the end of the visit.
