The inspector’s account

Licensing Program Analyst (LPA) Bernadette Allen made an unannounced visit to the facility to get a complaint amended that was processed on 8/23/2023 control number 56-AS-20230814142101.

During the visit LPA informed the administrator Michael Garcia about the corrections to original report. Michael signed the amended 9099 and 9099-C.

An exit interview was conducted where this report was discussed and a copy of the report was provided to Michael Garcia at the conclusion of the visit.
