Licensing Program Analyst (LPA) Javier Prieto arrived to the facility to have staff signed amended complaint investigation report #56-AS-20220707091011. LPA met with Maria Molina, Facility Nutritionist, and explained the complaint and purpose for signature. Both LPA Prieto and MS Molina signed a copy of the amended complaint investigation report and this Facility Evaluation Report to document visit and a copy of both reports were left at the facility.
