The inspector’s account

On June 27, 2025, the Department held an informal office meeting to discuss the status of all Aaspen Village Care facilities. In attendance was Regional Manager (RM) Leslie Mendiveles, Licensing Program Manager (LPM) Karen Clemons, Licensing Program Analyst, (LPA) Sarina Ramirez and Licensee Khan Mushtaq.

Per Licensee Khan Mustaq the property for Aaspen Village Care III has been sold, no residents have been residing at the facility since April 4,2024. The Licensee no longer has control of the property. As of 6/27/2025 the facility is closed.

An exit interview was conducted where this report was discussed and a copy provided to Licensee Khan Mustaq at the conclusion of the visit.
