Licensing Program Analysts (LPA) E. Conchas made an unannounced visit to the facility to conduct an annual inspection. At the time of the visit Diane Emerson, the Licensee, greeted LPA.
Upon entering the facility Diane informed LPA the last resident passed away in January of 2025 and has not had any residents since. LPA was informed the facility will no longer be operating.
LPA conducted an overall inspection of the facility. LPA observed no clients residing and no staff presently working and no furniture or clients belongings as well.
