Facility Evaluation Report
Licensing Program Analyst (LPA) E. Conchas visited the facility unannounced to complete a comprehensive annual inspection. LPA met with administrator, Criselda Abellanosa Miguel and explained the purpose of the visit.
Today’s inspection included a facility tour, record review. There were no residents due to being in day program.
The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected client bedrooms that are equipped with the required furniture such as mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately.
The hot water temperature tested within regulation between 105 -120 degrees F. The facility is equipped with operating smoke detectors and carbon monoxide detectors and with a fully charged fire extinguisher serviced on March 12, 2026. Posters such as personal rights, the CCL complaint poster, the emergency disaster plan, facility Sketch, grievances were posted in a common area.
Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to clients in care. All sharps were locked in kitchen drawers inaccessible to clients in care. There was a designated place for client/staff files. Overall, the facility appeared to be clean, in good repair, and operating in safe conditions for clients in care.
