Facility Evaluation Report
Licensing Program Analysts (LPA's) Edith Conchas and Paola Guerrero visited the facility unannounced to complete a comprehensive annual inspection. LPA Conchas and Guerrero met with Licensee Criselda Abellanosa Miguel and explained the purpose of the visit.
Today’s inspection included a facility tour, record review, and interviews with staff and no residents due to being in day program,.
Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected client bedrooms: they are equipped with the required furniture such as mattresses, nightstands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately.
The hot water temperature tested within regulation between 105 -120 degrees F. The facility is equipped with operating smoke detectors, carbon monoxide detectors and is fully charged fire extinguisher. Posters such as personal rights, the CCL complaint poster, the disaster plan, facility Sketch, and evacuation plan were posted in a common area.
Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to clients in care. All sharps were locked in kitchen drawers accessible to clients in care. There was a designated place for client/staff files. Overall, the facility appeared to be clean, in good repair, and operating in safe conditions for clients in care.
