Facility Evaluation Report

On August 6, 2026, Licensing Program Analyst (LPA) E. Conchas, made an unannounced case management visit to the facility to deliver an amended report on complaint number 56-AS-20260624123648.

LPA met with Assistant Administrator, Maria Cervantes and explained the reason for the visit.

During today's visit, LPA conducted a tour of the facility for a Health & Safety Check.

Based on today's observations made, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted where this report was discussed, and a copy was provided to Administrator Cervantes.
