Facility Evaluation Report

Licensing Program Analysts (LPA) Edith Conchas conducted an unannounced visit to the facility for the purpose of a Case Management Visit on complaint numbers 56-AS-20260420105511 , 56-AS-20260612115850 , 56-AS-20260624123648 . LPA identified thyself to Administrator assistant Maria Cervantes and discussed the purpose of the visit.

LPA interviewed staff and residents in care in addition to doing a walk through around the facility. No imminent health and/or safety hazards inside or outside the facility were observed. LPA observed staff present at the facility providing care, activities and maintenance updates.

Based on today's observations made, no deficiencies were cited per Title 22, of the California Code of Regulations. An exit interview was conducted where this report was discussed, and a copy was provided to Administrator assistant Cervantes.
