Complaint Investigation Report

Interviews with staff and resident's deemed that R1's personal rights were not violated and staff are helpful. Due to the information obtained, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated at this time.

A finding that the complaint is UNSUBSTANTIATED means although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.

An exit interview was conducted where this report was discussed with and a copy was provided to Krista Overymyer .
