Facility Evaluation Report
On 11/04/2025 at 9:15AM Licensing Program Analyst (LPA) Renese Howell-Small arrived unannounced to the facility in order to conduct a case management visit regarding Unusual Incident Reports (SIR). LPA conducted this visit jointly with facility liaison, Kenya Figueroa from Inland Regional Center. LPA met with Co-Administrator, Shay Bickham.
LPA reviewed staff schedules, client behavior plans and conducted staff interviews regarding the implementation of client behavior plans and also reviewed documentation to clear the facility's Plan of Correction(s).
LPA observed two clients in care and three staff at the facility. The facility had sufficient staffing at the time of the visit and no deficiencies were cited.
An exit interview was conducted where this report LIC809 was discussed and copy was provided to Co-Administrator, Shay Bickham.
