The inspector’s account

On 08/19/2025 at 11:30AM Licensing Program Analyst (LPA) Renese Howell-Small conducted an unannounced visit to conduct a case management visit to follow up on several Unusual Incident Reports submitted by the facility. LPA was greeted by staff and granted entrance to the facility. Administrator, Eddie Bickham was contacted via telephone and informed of the purpose of the visit. The Administrator arrived at the facility during the visit.

LPA conducted a brief tour of the facility and observed two (2) clients in care. The facility was clean, a comfortable temperature and there was adequate staffing. LPA observed sharps, medication and chemicals/cleaning supplies to be locked and inaccessible to clients in care.

LPA conducted interviews with four (4) staff and two (2) clients and reviewed staff and client files. Staff stated that they are trained on a regular basis and feel confident to perform their job duties efficiently. Staff have become familiar with the clients and their behaviors and have recently completed a training conducted by a licensed behavioral professional. The two (2) clients interviewed were observed to have a happy disposition at the facility and towards staff and could communicate their needs effectively.

No deficiencies were cited during this visit. An exit interview was conducted where this report LIC809 was discussed and a copy was provided to staff, Bridie Holmes.
