Facility Evaluation Report
On 07/22/2026 at 9:20AM Licensing Program Analyst (LPA) Renese Howell-Small arrived unannounced to the facility in order to conduct a joint Case Management visit with Consumer Program Liaison (CPL), Kenya Figueroa with Inland Regional Center (IRC). LPA observed one (1) client in care and four (4) staff. Two clients were out in the community.
LPA interviewed staff, reviewed staff training, staff schedule, client notes and completed a tour of the facility. LPA observed that two doors were replaced for the shared client bathroom and the empty client bedroom next to the shared bathroom.
LPA advised the House Manager that the Licensee/Administrator Eddie Bickham and Chala Lolles would be contacted to schedule a tentative meeting.
No deficiencies were cited during this visit. An exit interview was conducted where this report LIC809 was discussed and a copy provided to House Manager, Bridie Holmes.
