Facility Evaluation Report
On 05/19/2026 Licensing Program Analyst (LPA) Renese Howell-Small arrived unannounced to the facility in order to conduct a case management visit related to an incident that involved misconduct between three (3) staff and a client. LPA met with House Manager, Bridie Holmes. LPA observed three (3) clients in care.
LPA interviewed two (2) staff and reviewed staff records. Based on the incident report that was submitted to Community Care Licensing (CCLD) and staff interviews, the three (3) staff involved in the incident are no longer employed by the facility. On 05/08/2026, the Licensee/Administrator conducted a staff training on professional conduct, client rights and staff reviewed and signed the facility's policy on substance abuse.
No deficiencies were cited during this visit. An exit interview was conducted, where this report LIC809 was discussed and copy was provided to House Manager, Bridie Holmes.
