Facility Evaluation Report
On 04/17/2026 at 9:30AM Licensing Program Analyst (LPA) Renese Howell-Small arrived unannounced to the facility in order to conduct a Case Management visit regarding an incident that was reported to Community Care Licensing Division (CCLD).
LPA interviewed client(s) and three (3) staff. LPA requested the staff schedule, reviewed the staff files for current certifications and trainings and reviewed client files. The staff training log contained a sign-in sheet for facility staff who participated in the Crisis Prevention Institute (CPI) training conducted in 12/2025.
No deficiencies were cited during this visit. An exit interview was conducted where this report LIC809 was discussed and a copy provided to House Manager, Bridie Holmes.
