Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria conducted an unannounced case management visit to follow up on an incident report sent to licensing dated on 2/17/26. LPA was greeted and explained the reason for the visit to Assistant Manager, Jahaira Valdez and staff, Deynshona Smith.
Per the incident report, Crisis Prevention Intervention (CPI) was utilized on a client who was in behavior. LPA reviewed facility records and observed that staff documented the incident on an IRC Special Incident Report and on the client's notes. The facility was under the impression that by reporting it on the IRC portal, they were also reporting it to CCL. LPA educated the licensee, administrator and staff present on SIR reporting requirements and manual restraint or seclusion review procedures.
No deficiency and one technical assistance was cited during this visit. An exit interview was conducted where this report LIC809 and LIC9102TA was discussed and provided to Assistant Manager, Jahaira Valdez.
